Listed below is the required paperwork for newly hired and return employees for Catholic Charities Camp Happiness.
Please read carefully. There is a Key below to help you know which forms you need to complete according to if you are a “new” or “returning” staff. There is also a section for ALL staff. Some are trainings and you will be asked to complete a test, while others require a signature only.
Again, please read fully and carefully. We have tried to make this as easy as possible – email Lauren if you have any questions.
KEY (please note which forms you need to complete according to if you are a new staff member or a returning staff member; there are also forms for all staff)
N = NEW STAFF ONLY
R = RETURN STAFF ONLY
A = ALL STAFF
(N) Read the Bloodborne Pathogens handout, then print out and complete the test, sign and return.
(N) Read the Safety in the Workplace handout, then print out and complete the test, sign and return.
(N) Read the Standards of Conduct for Ministry, then print out the acknowledgement, sign and return.
(N) Read the Catholic Charities Statement of Ethics, then print out page 3, sign the bottom and return.
(R) Read the Health & Safety Refresher Course, then print out and complete the test, sign and return.
(R) There is no need to print out the Employee Handbook – it is too many pages. But please print out and sign the acknowledgement form that you have received the Employee Handbook.
(R) Complete the following only if info has changed from last year. For example – you have a new bank account, complete the Direct Deposit form; a new address, complete the Employee Data Sheet
ALL STAFF (A)
(A) Read the Conflict of Interest Statement and then print out the Conflict of Interest signature page – Sign the right side if you feel that you are NOT in conflict. If you do feel you have a conflict of interest, sign the left side and explain in the space provided.
(A) Print out and sign the Hepatitis B Vaccination Form – even if you’ve done so in the past, you must sign again. Choose if you will receive the Hep B Vaccine or not.
(A) Read the Policy for Safety of Children in the Matters of Sexual Abuse and then print out, sign and return the signature page.
(ALL) Fingerprinting (BCII & FBI)
Must be done during the week of May 6-10 between the hours of 9:30-4:30. There is no need to call for an appointment if you come during this time. You will come to St. Augustine Manor to get your fingerprints captured so that they can be sent to the Ohio Bureau of Criminal Identification and Investigation (BCII) & to the FBI. When you arrive at St. Augustine Manor, tell security and the receptionist you are here to meet with Angie in the Catholic Charities HR office for fingerprints. The HR office is on the ground floor by our Disability Services offices. So if you have finished your paperwork, you can bring that as well.
***Please make every effort to come that week of May 6. HR is very busy and they have set aside this time for us. We must have your fingerprints taken before camp begins or you cannot start work. If you cannot make it during that week, please let me know and we can make other arrangements.
- You must bring your driver’s license or state ID with you to your appointment.
- You will need to know your social security number as well.
- Disclose any convictions or guilty pleas (If you do not disclose this and your results come back showing an incident, that may be cause for termination).
- You are expected to have your fingerprints completed by between May 6-10. Any exceptions must be before Friday, June 3, 2019.
(A) First Aid / CPR Certification – If you have current certification in First Aid and/or CPR, please send us a copy of that certification. We will be offering new certification courses and refresher courses.
NEW & Second Year Staff
VIRTUS / Protecting God’s Children
All staff working or volunteering for a Catholic organization must complete Virtus. Virtus is a training designed to help us “protect God’s children” from abuse. The initial training will be a video featuring interviews of those who have been abused and those who have abused. Please be aware that while it can be difficult to watch, it is also very necessary, and required.
New Staff – You will receive Virtus training during camp orientation. However, if you have completed the Virtus training elsewhere, please provide us with a certificate of proof. There is no need to read the articles below or complete the answer sheet until your 2nd year of employment.
Returning Staff in their 2nd Year ONLY – Read all 24 Articles (no need to print them all out). Print out the Answer Sheet. At the end of each article there are questions. Mark the answer to these questions on the answer sheet for the corresponding article. You will then turn the answer sheet in to your supervisor.
If you completed the above articles online over the last year, you will need to provide proof. If you did not, then get reading!!!
Returning Staff in their 3rd year or more –Your Virtus obligation is complete!
You Are Finally Done!!!
When you have completed each test/form, make sure your first and last name is printed legibly, that your Service Site is indicated as Camp Happiness and that your Supervisor (Lauren Mailey) is also indicated. Make sure you have signed everywhere you need to! Then forward to Lauren!
